Toottip Pet Problem

Hello, im making some changes in the New Pet Sistem, this sistem btw:

 

And in the tooltip i want something to show the Pet Name / Evolution Form,

here my work: 

 

In Label "Mini Macaco" Green i want it show the name of the pet by sql:

 

And where show "Jovem" this is the evolution form but i created this with

but this read the pet level and give a evolution name but i don't want like this, i want to pick this: 

something like petevolution[1] == 0 :

self. bla bla bla (you know i guess)

 

Can you give me some lights? where my work code:

 

btw in in if attrSlot[5][0] this must pick the evolution level but don't "/
