Problem in syserr

Channel syserr issue

 

I'm having problems in the syserr of the channels could someone help me?

I've tried everything.

Seems you're running simultaneous queries for affects.

[camboja41 0]

This and because of this error the potions that are activated and when teleport disappear. This problem is in the affect table of the navecat.
So I was wondering how do I solve this problem.
